This Partner Incentive Claim Form is provided by InStride Surfaces Inc. (“Company,” “we,” “our,” or “us”) for use by authorized retail partners and their sales associates (“you” or “your”). By submitting a claim, you acknowledge and agree to the Terms & Conditions, Disclaimers, and Acknowledgment outlined below. Please review all information carefully prior to submission.
Terms & Conditions
Use of this incentive program is subject to the following terms:
- Eligibility for Payment: Claimed sale(s) must be at least 30 days past the invoice date and reflect a status of “Paid.”
- Submission Window: All incentive claims must be submitted within 15 days after the invoice is paid and has reached the 30-day mark.
- Payment Method: Approved incentive payments are issued directly to the Retail Sales Associate (RSA) via ACH direct deposit or Zelle.
- Processing Time: Please allow up to 60 days from the date of submission for approved payments to be processed.
Disclaimers
Please review the following important information regarding claim submission and approval:
- Additional documentation may be required to verify eligibility and validate submitted claims
- Only one RSA claim may be submitted per qualifying invoice or individual sales line item
- Submission of a claim does not guarantee payment and remains subject to review and approval
- InStride Surfaces reserves the right to delay or deny payment if submitted information cannot be verified
Acknowledgment & Agreement
By submitting this claim, you confirm and agree to the following:
- You are currently employed by the partner listed on this claim during the applicable incentive period
- You personally participated in the sale(s) associated with this claim and have complied with the applicable SPIFF agreement referenced in the form
- You understand that total incentive payments of $600 or more within a calendar year may be subject to IRS 1099 reporting requirements
